When reading tarot cards, what does an up-side-down card mean?

It is the opposite of a right-side-up card? Such as arrogance or vanity to a card that would otherwise mean triumph, pleasure, gain etc. ??? And more imporantly - are they read as up-side-down when facing the reader or the querent?
